Summary Of LifeNet Employee Benefit Information

Following 90 consecutive days of full-time employment with LifeNet, an employee becomes eligible to participate in a broad range of comprehensive benefits. Participation is mandatory in non-contributory insurance coverages (those paid 100% by LifeNet). Participation is voluntary in contributory coverages (those paid in part or full by the employee).

Listed below are brief descriptions of the various benefits available to full-time LifeNet employees. For complete details on company benefits, please contact the Human Resource Department.

Employee Amounts in multiples of $10,000 not to exceed 300% of the employee's annual salary amount.
Spouse Amounts in multiples of $10,000 not to exceed 50% of the amount the employee purchases on his/her own life.
Child(ren) Only $5,000 or $10,000 on each child.

* These coverages are 100% employee paid.

Employee Self Any amount between $25,000 & $250,000 in increments of $25,000.
Dependent Spouse 50% of the employee amount
  Child(ren) Only 10% of the employee amount
  Spouse & Child(ren) 40% of the employee amount on the spouse & 5% of the employee

* These coverages are 100% employee paid.

Medical Reimbursement Account: The money placed into this account may be used to cover allowable unreimbursed medical expenses (including dental & vision expenses). The amount redirected into this account is limited to $3000 per year maximum.

Dependent Care Reimbursement Account: The money placed into this account may be used to cover the cost of childcare expenses for dependents. The amount redirected into this account is limited to $2500 per year maximum.

Employees are encouraged to participate in LifeNet's Section 125 Premium Only Plan, which allows an employee to pay most contributory insurance costs with pre-tax dollars. AFLAC Insurance Company is currently administering this plan.
