LifeNet’s Community Health Paramedic program will provide State Licensed, advanced trained Paramedics to respond on a non-emergency basis. This program will be able to provide advanced patient assessment, bedside laboratory tests and 12 lead EKGs. It will allow LifeNet to work with Hospitals, ACOs , Home Health and Hospice to help keep patients at home, where they can feel safe and manage their condition outside of the hospital.

Our Community Health Paramedics will be able to help identify an individuals SDOH (social determinants of health) and determine what needs are not being met, and who needs further assistance. The Community Health Paramedic program’s whole goal is to connect them with existing community resources to help bridge the gap for them medically. CHP Medics will work with primary care providers and other specialists to help individuals better manage their health care. Some of these resources include helping with healthcare appointments, food resources and many other available community and government assistance in our area.

LifeNet’s CHP program will ultimately help patients who want to remain in their homes, avoid unnecessary trips to the ER, and improve their quality of life. We are excited to bring this program to our community and proud to offer such a helpful resources to Texarkana and the surrounding areas.
